General Information
Office Hours/Appointments
Our office is open generally between
8:00 a.m. and 5:00 p.m., Monday through
Friday. Patients are seen by appointment
only.
Registration
For initial appointments with our
practice, patients should arrive at the
clinic 15 minutes prior to their scheduled
appointment time. If you are a new patient,
we ask that you complete a New Patient
packet. These forms provide
valuable information for the physician
and will enable us to establish a history
and administrative file for you. If you have any questions regarding
the information requested, please ask
for assistance to complete the form as
soon as possible. After arriving, please
check in with our patient coordinator.
If you are an established patient and
information concerning your insurance,
your address, phone number, or place of
employment has changed, please let us
know so that we may update our records
accordingly.
Lodging
We do our best to expedite patient care and coordinate additional testing. Patients coming from a distance may want to consider an overnight stay to complete their evaluation. The office can provide you with a list of accommodations in the surrounding area.
Record Changes
It is the patient's responsibility
to notify the clinic of any personal record
changes such as name, address, phone number
and insurance information. However, to keep our records
current, all patients will be asked to
complete new paperwork every six months.
Medications
Presciptions and refills will be issued
only during office hours and until 2:00 PM on Friday.
Appointment Cancellations
Appointments should be cancelled within
24 hours to allow other patients an opportunity
to be seen.
Insurance Information
Insurance Copays
Insurance copays are expected at the
time of service. Copays cannot be waived.
A $28 charge is added to any returned
check.
American Express, Visa, Mastercard, Discover and Diners Club accepted.
Referrals
In order to serve our patients in a timely and efficient manner, patients must provide our clinic with all required referral documentation from their primary care physician.
Insurance Changes
It is the patient's responsibility
to notify the clinic of any insurance
changes. A photocopy of the patient's
current insurance card will be kept in
the file to ensure prompt and accurate
filing.
What to bring to your appointment
- New Patient Packet completely filled out.
- All X-rays, MRI, CT Scan, Bone Scans along with reports.
- If any lab work was performed, all results
- If you had a biopsy done please bring the slides and the report.
- Please bring all medical records from other doctors you have seen for this condition.
- Insurance card.
